Yokosuka Move-Out Housing Process

On-base housing - Move Out Process

 

Step 1 Schedule a pack-out date

Upon receiving PCS orders, schedule a pack-out date with FISC Outbound Personal Property Office located at Bldg. 5013, DSN 243-7061 with a copy of your orders. Go to Step 2 once you have received your HHG pack out dates.

Step 2 Intent to Vacate form

Visit your local Housing Office with a copy of your orders to fill out an Intent to Vacate Form at least 30 days prior to the estimated vacate date, preferably upon receipt of detaching orders. Approximate dates are acceptable for the purpose of completing the vacate forms. Once you receive a confirmed flight date please contact your local Housing Office to update your Intent to Vacate form to schedule your final inspection.

Ikego: 246-8027

Negishi: 242-4250

Step 3 Final Check-Out Inspection

Attend the scheduled appointments made in Step 2. Command Check-out card must be signed at the Final Inspection.

Step 4 Reinstate OHA/LQA

Military: Upon clearance of government quarters, you will receive 2 copies of BAH Memorandum and 2 copies of Clearance slips to submit to detaching and gaining Disbursing Office.

Civilian: Upon clearance of government quarters, you will receive a copy of Military Pay Order (DD-114) and a Clearance slip to submit to HRO Yokosuka.

Important

Termination of Telephone and Internet:

Contact the Base Communication Office (BCO), Bldg. C-20, room 103 at DSN 243-5847 with a set of your new PCS orders. Please be advised that final bills require two (2) business days to process.

Termination of Military Access Service (long distance) & Cellular Phone Service:

Contact AT&T at 241-4242 located in the NEX Car Rental Building #J-196.

Renting pots, pans and bed sheets:

Contact the Community Center at 243-6713.

Navy Lodge Reservations:

We recommend you make reservations as early as possible since your desired TLA/TQSE accommodations may be full. One-day maximum temporary lodging allowance is authorized unless arrangements for departure transportation dictate lodging requirements of more than one day. For pet owners, Contact the Kennel for reservations at 243-4530.
 

Off-base housing - Move Out Process

 

Step 1 Visit FISC Outbound Personal Property Office

Upon receiving PCS orders, call DSN 243-7061 to schedule an appointment with FISC Outbound Personal Property Office with copy of your orders to schedule HHG pack out and move dates. Go to Step 2, once you have received your HHG pack out dates.

Step 2 Contact Housing Services Center

Call the Housing Services Center at DSN: 243-8834, 8848 or 9037/ commercial: 046-816-8834, 8848, 9037 to schedule your termination appointment with a counselor.

** The termination appointment date should be scheduled at least 10 days prior to your check out inspection as stipulated in your lease agreement. If you are in Rental Partnership Program (RPP) unit, 30 days prior vacate notice is required.

Step 3 Scheduled Appointment

Visit the Housing Services Center (Bldg. 1441) with a set of new PCS orders for your appointment made in Step 2.

As part of your termination the following will be scheduled during this appointment as applicable:

1. Disconnection of Navy loaner appliance by Tokyo Gas (fee to Tokyo Gas: ¥4,000 - ¥15,000)

2. Navy loaner furniture/appliance pick up

3. Termination of utilities (gas, electricity, water – you will be required to pay amount based on meter readings)

4. Final/Check-out inspection with agent/owner (you may request a housing inspector be present)

Note: You (or your representative with a Power of Attorney) must be present at your off base home for the above scheduled appointments.

Step 4 Final Check-Out Inspection

Attend the scheduled appointments made in Step 3.

Step 5 Statement of Release

Upon clearance of the final inspection, both you and the agent/owner will sign the ‘Statement of Release’ form and copies must be provided as follows:

1. Pink copy to the agent/owner

2. Yellow copy to the Housing Services Center

3. Original…

Military - to PSD or Ship’s Disbursing Office to terminate Overseas Housing Allowance (OHA)

Civilian – to your HRO/Finance Office to stop Living Quarters Allowance (LQA)

Note: Security deposit will be refunded when your off base home is cleared. Damages and/or cleaning fees (if required) will be deducted from your security deposit and the agent/owner will indicate the amount of refund on the Statement of Release form. If you do not agree with the amount, please do not sign the Statement of Release. Please contact the Housing Services Center.
