    FACT SHEET 

    All, USS George Washington will once again be holding a Friends and Family Day Cruise. Below are details for you to pass along in preparation for this great event. As always, please remember OPSEC and ask that you avoid from posting dates and times on the Internet.

    Important information:

    Price: $15 per guest 

    Guest limit: 5 guests per Sailor

    Sponsor: You must have a sponsor aboard George Washington to attend this event. This is not open to the general public. 

    Registration: Registration begins 11July and ends 20August. 

    You must register and be approved by the Medical department before you can purchase tickets. 

     No tickets will be sold after 20August. 

    Registration forms will be posted on www.gw.navy.mil on 11 July.

    Registration:

    1.) Family Registration Form: A separate “Family Registration Form” must be filled out for each adult. For children, the form must be signed by the parent or legal guardian. Please do not include children with different parents/legal guardians on the same form.

    2.) Medical Review & Emergency Data Form: A separate form must be filled out for each person attending for the Friends & Family Cruise.

    3.) Medical Care Power of Attorney: Only required if you will be escorting a minor child for whom you are not the legal guardian. Please contact your local legal office to obtain appropriate document(s).

    Minimum age: 8-years old (no waivers or exceptions) 

    Childcare: USS George Washington will not be coordinating any childcare. Caring for children ineligible to attend is the sole responsibility of individual parents.

    Baggage: Besides a small-medium size woman’s purse, no backpacks, duffle bags, camera bags or luggage is authorized 

    Clothing: Mini-skirts, high heeled shoes and sandals are prohibited. Wear only closed-toe shoes.

    Medication: Guests are encouraged to bring a two day supply of all required medication. Those prone to mild motion sickness should consult a doctor and bring medication. Sun screen is also strongly recommended.

    Expecting Mothers: For their safety, women who are pregnant are not permitted to attend the Friends & Family Day Cruise. 

    Photography: Photos are not only authorized, but encouraged. Only in certain designated areas is photography off limits. Those areas will be indicated with clearly marked signs. 

    Smoke: Smoking is only authorized on the fantail. 

    Island access: Only guests with VIP badges are authorized access to the island. The island above the flight deck is off limits to all others.

    Escorts: Guests are required to be with their Sailor at all times. If the Sailor has to go on watch, a secondary sponsor will remain with the guests

    Individuals from the following countries must obtain written permission from Commander, Fleet Activities, Yokosuka (CFAY) prior to entering the base. 

    A sample letter request may be picked up at the CFAY Security, Industrial Security Office (ISO) or at CFAY Main Gate.

    Sign Up Process

    1. Complete Family Registration Form
    A separate form must be filled out for eachadult attending the Friends and Family Day Cruise, though children can be included on their parent’s forms. Forms are NOT required for the active duty member assigned to USS George Washington or embarked Commands (CTF-70, CVW-5, DESRON-15). The form must be signed by one parent/legal guardian of the child(ren) listed on the form. Please do not include children with different parents/legal guardians on the same form.

    2. Complete Medical Review & Emergency Data Form
    A separate form must be filled out for each person attending the Friends and Family Day Cruise. Forms are NOT required for the active duty member assigned to USS George Washington or embarked Commands (CTF-70, CVW-5, DESRON-15). Active duty members from other Commands ARE required to complete this form.

    Once completed, these forms can be submitted one of three ways:

    a) Scan the forms and e-mail them to: gwfamilycruise@cvn73.navy.mil

    b) Drop the forms off at Patriot’s Landing (Building 48, Naval Base Yokosuka) or CVW 5 Front Duty Desk (Hanger 1401 NAF Atsugi)

    c) Mail the forms to:

    3. Review Legal Note and complete Medical Care Power of Attorney (MCPOA)

    ****ONLY REQUIRED IF BRINGING SOME ELSE’S CHILD****
    Only
    required if you will be escorting a minor child for whom you are not the legal guardian. The MCPOA MUST be signed by the child’sparent or legal guardian AND notarized. These forms must be physically droppedoff at Patriot’s Landing or CVW 5 Front Duty Desk or mailed to the above addresses and received no later than 20-AUG-2011.

    4. Make payments at MWR Booth onboard USS George Washington

    Once steps 1, 2 and 3 are completed, the Sponsor onboard USS George Washington should pay for the Friends and Family Day Cruise ($15 for every attendee, other than the sponsor). A list of attendees whose packages are complete will be forwarded to MWR daily and payment can be made once MWR is notified that all forms are received and verified.

    GENERAL RELEASE & CONSENT TO EMERGENCY MEDICAL DENTAL CARE

    Must be filled our for all Friends and Family Day Attendees not assigned to Commands onboard USS George Washington (including active duty members from other Commands). All friends and family members of a given sailor may be included on the same sheet. If additional space required, attach additional sheets.

    NOTE: This is a general release of liability, which, when executed, becomes a binding document releasing the United States Government, the Department of Defense, the Department of the Navy, Commander Seventh Fleet, Commander Carrier Task Force 70, Carrier Air Wing Five, Destroyer Squadron Fifteen, USS George Washington (CVN 73), and all officers, crew, employees, agents, and authorized representative of any of these agencies, entities or units, from any and all liability whatsoever arising from injuries to you and/or your minor child or ward, as well as damage to or loss of property, while embarked on board USS George Washington (CVN 73). Before signing this release, you should read this document carefully and consider all possible implications and associated risks prior to signing it. If you wish to give permission for your minor child or ward to participate in the Friends and Family Day Cruise, you must also complete the embark Authorization form appended hereto.

    I and/or my child(ren) or ward(s), listed below, am/are/is embarking as a passenger on board USS George Washington (CVN 73) in the month of August, 2011. The purpose of this embarkation is voluntary participation in the event commonly known as the “Friends and Family Day Cruise.” I hereby confirm that participation in this event is strictly voluntary and done knowingly, intentionally, and of free will, having considered all of the attendant risks associated with the Friends and Family Day Cruise, of which I have been fully informed. I understand that because USS George Washington (CVN 73) is an industrial environment, there is a heightened risk of injury on board, especially while the ship is at sea. I specifically and expressly request permission for me and/or my child or ward to embark on board USS George Washington (CVN 73), for the Friends and Family Day Cruise. In consideration of the permissions extended to me and/or my child or ward to participate in the Friends and Family Day Cruise, I hereby knowingly, intentionally, voluntarily and forever release the United States Government, the Department of Defense, the Department of the Navy, Commander Seventh Fleet, Commander Carrier Task Force 70, Carrier Air Wing Five, Destroyer Squadron Fifteen, USS George Washington (CVN 73), and all officers, crew, employees, agents, and authorized representative of any of these agencies, entities or units, from any and all claims, actions or causes of action arising out of death, injury, and/or any damage, destruction, or loss of any property belonging to me and/or my child or ward. Emergency care is the only type of care authorized. Guests with chronic or incipient medical problems should be aware of the limitations of extended care available afloat. Transportation to a definitive care facility may be required as an adjunct to authorized emergency medical or dental care. Personnel receiving medical or dental care who are not otherwise eligible to receive such care may be obligated to reimburse the U.S. Government.

Persons who have experienced heat exhaustion or heat stroke are strongly cautioned to drink plenty of fluids both before and during the cruise.

    Legal Note

    ATTENTION GW SAILORS:

    If you are planning to bring a minor (age less than 18) who is not your child on the Friends and Family Day Cruise, then you will be required to have a medical care Power of Attorney from that minor's parent/lawful guardian allowing you to authorize medical treatment for the minor. Please contact your local legal office to obtain appropriate document(s). It may be notarized anywhere there is a civilian notary public, such as at a bank or a military legal assistance office (so long as the parent/lawful guardian is eligible to receive military legal assistance).

    This form must be completed, properly notarized, and received by the Friends and Family Day Cruise Committee no later than 20-Aug-2011.

    It is your responsibility to ensure that a Medical Care Power of Attorney is executed and received prior to the deadline.

    It is extremely important that you meet this deadline and submit the Power of Attorney well in advance of the cruise.

    Remember

    · GW’s Legal Department cannot notarize aPower of Attorney for aperson who is not eligible to receive military legal assistance.

    · A Power of Attorney cannot be notarized without the presence of the individual signing and granting the Power of Attorney.

    Dates have been removed. I will tell you, that all this information was copied from the official GW website. Unfortunately, the people that wrote this document, are not following their own guidance.

    Much better :-) I am sure the information can still be found online but at least it is not posted HERE which was my point. Just because classified/OPSEC material is available from other sources does not mean that you can repost or retransmit it (i.e. Wikileaks). Yeah I saw it was on the main page of the GW website as well, people are getting sloppy because now I know something about a ship's operations that in my position I should not be knowing.

    Wow that seems like alot of paper work. When we did these in VA on my ship. Nothing but an ID was required and you could almost go anywhere on the ship.

    George Washington Takes Friends and Families On Cruise

    More than 2,000 family members and friends of the crew of the nuclear-powered aircraft carrier USS George Washington (CVN 73) enjoyed a Friends and Family Day cruise, Aug. 27.


    Family and friends of Sailors stationed aboard the nuclear-powered aircraft carrier USS George Washington (CVN 73) board the ship prior to the ship’s Friends and Family Day cruise into Tokyo Bay Aug. 27. The Friends and Family Day cruise allowed Sailors’ families and friends to board George Washington to get a firsthand look at what life for their Sailor is like at sea.     George Washington, the U.S. Navy’s only full-time forward-deployed aircraft carrier, provided multiple entertainment acts, including a famous Japanese Elvis tribute performer, Taiko drummers, and Hawaiian dancers, along with paintball, bouncy houses, movies, human gyroscope, sumo wrestling, and face painting, as well as various entertainment provided by the ship’s Morale, Welfare and Recreation (MWR) department.

    The day was highlighted by a sea-power display and air show involving the aircraft and pilots of embarked Carrier Air Wing (CVW) 5.

    “This cruise is basically a way to say thank you to the families that sacrifice their Sailors to us for six months,” said Lt. Cmdr.

    Dave Hecht, the Friends and Family Day cruise coordinator. “We gave our families access to our ship, to see how we live and work underway, as well as show them a demonstration of our air prowess. This is the least we could do for their sacrifice.”

    George Washington pulled out of Yokosuka at 7:30 a.m., allowing the families to spend a maximum amount of time aboard with their Sailors. The ship’s MWR department ran the entertainment in the hangar bay, while the ship’s Supply department supplied the guests with food, beverages and several tables that sold George Washington merchandise not otherwise available for sale.

    “This cruise was a huge team effort; every department had a hand in setting this cruise up,” said Hecht. “Every person contributed a piece of the puzzle to make this event a success, and because of everybody’s hard work, the puzzle has become a masterpiece.”

    “This was a great visit, and I especially liked that George Washington provided a lot of activities inbetween the events for the kids so they didn’t lose steam,” said retired Chief Navy Counselor Jon Way, scout leader of Boy Scout Troop 13, who brought several of his scouts aboard George Washington for the cruise.

    “But the air show was definitely the highlight of my scouts’ visit.”

    At the middle of the cruise, everybody moved to the flight deck, where everybody got to watch CVW-5’s aircraft launch off the flight deck, fly in formation and perform touch and goes, a maneuver where the aircraft makes an approach as if making an arrested landing but then continues back into flight.

    “This was an absolutely amazing time,” said Tomoko Asano from Tokyo, a guest aboard George Washington. “I was overwhelmed by everything, especially the air show. I kept saying ‘wow’ all the time, I was that amazed.”

    The cruise, while not simply held to thank George Washington Sailors’ families, was also held as to say thank you to their host nation, said Hecht. As a result, several Japanese citizens also visited the ship for the cruise, both military and civilian.

    “We have 22 Japan Maritime Self-Defense Force master chief petty officers aboard for this cruise,” said George Washington Command Master Chief Martin King. “They always enjoy coming aboard to see the ship, and they always enjoy seeing our Sailors at work.”

    George Washington pulled back into Yokosuka in the early evening, with many guests saying how much they enjoyed the event.

    “This cruise was absolutely fantastic,” said King. “While talking to my Sailors and their families, I know that it was a rewarding experience for the families to see what their Sailors do and how they live while at sea.”

    George Washington’s mission is to ensure security and stability in the western Pacific and to work with regional partners to respond to any crisis across the operational spectrum.

    Im so excited for this next year! It looks like so much fun. I think its great for families to have a chance to see where their loved ones stay and work. Theres so much information that spouses aren't privvy too, so this is a good middle ground to let us into the boat life a little more. Love all the pictures too!!
