American Hospital Services Group LLC is currently seeks a Registered Nurse Case Manager to work as a civilian contractor at Kadena Air Base in Okinawa, Japan.

The 18th Medical Group, the Air Force's largest freestanding ambulatory clinic in terms of assigned members (500), provides extensive medical, dental and support services to Kadena and other Department of Defense members on Okinawa. More than 60 medical and dental providers care for nearly 500 patients each day. Services include family practice, pediatrics, gynecology, optometry, mental health (including adult and child psychiatry) and physiological training, and a health and wellness center.

Non local candidates will be provided relocation reimbursement, monthly housing allowance, sign on bonus, and CME.

Schedule:

7:30am – 4:30pm Monday through Friday No Call or Weekends.

Description:

The RN Case Manager will provide case management, care coordination and discharge/disposition planning for inpatient and outpatient care settings. Assists service members and/or veterans, family members and caregivers with receiving the most appropriate options and services to meet their complex needs.

Requirements:

BLS Certification

- Knowledge of the core case management functions:
*Assessment
* Planning
* Facilitation
* Advocacy

- Must be skillful and tactful. Knowledge of HIPAA, AAAHC, and TJC.

- Experience with McKesson (Interqual) and/or Milliman Care Guidelines desired.

Education:

- Graduate of nationally accredited Associates Degree, Diploma, or Baccalaureate Degree program in nursing. Baccalaureate is preferred.

- Active, valid, current and unrestricted RN license.

- Shall have at least 4 years of active nursing practice, 18 months of which must be in discharge planning or clinical case management for adults, children, families, seniors or groups. Must have worked in nursing the past 12 months. Home Health exp. desired.

- Certification in Case Management is desired.
