    U.S. Embassy Passport Services Yokosuka Base Site Visit

    U.S. Embassy Passport Services will be at the Fleet and Family Support Center Dec. 3 from 10 a.m. to 3:45 p.m., receiving appointment only renewal and first-time passport applications.

    To schedule an appointment, FAX your information (name, phone number, e-mail address and desired appointment time) to the U.S. Embassy at 224-5914 and they will contact you with an appointment time.

    Passport applicants must bring all required paperwork and must provide their own photocopies of proof documents.

    Photocopies can be made at the library or at the Navy Exchange Pack and Wrap Center.

    Has anyone done this before or has made an app. for this Friday? My husband made an app. for us and I tried to do the paperwork today and am confused. The page he was emailed says we need to fill out the DS 11 form, specifing BAR CODE must appear on the UL of the application form and to fill out the form online. I filled it out online and I cannot print it. When I DO print the form that is printable the bar code does not show up in the UL side of the page. My husband emailed but we won't get a response until tomorrow, if at all. Can anyone help?
